Project Summary:

Horizon Meds Manager™ (HMM) is a proven pharmacy information management solution that captures details of medication used in both inpatient and outpatient settings. It also links encounters in order to create a patient medication profile across facilities. The solution meets the changing needs of today's pharmacist by integrating with the physician order entry (POE) application.

This eliminates the need for any manual transcription and helps the pharmacy to electronically receive, verify and send medication orders to automated dispensing and administration applications thereby ensuring patient safety. Medication Management of the patient is taken care in HMM by assurance of patients' Five Rights (right patient, right medication, right time, right route and right dose). HMM also allows pharmacists to maximize their productivity and play a more active role in patient care.
